At OnDemand Care Services, we believe that every adult in England and Wales has the right to good quality housing and a place to call home. Our mission is to provide the people we support with a home for life – a home where they can live as independently as possible in a safe and supportive environment.

Ondemand Care Supported Living provides comprehensive semi-independent and 24-hour  supervised living services, offering support and accommodation to Look After Children  and Care Leavers, aged between 16 and 25. Services offered will be based on an  assessment of the care leaver’s needs and be part of the pathway plan for them. 

The young people we accommodate and support may have varying needs which may include: 

  • Autistic spectrum disorder
  • Attention deficit disorder
  • Mental health issues
  • Challenging behaviour
  • Moderate learning disabilities
  • Drug and alcohol abuse
  • Self-harming behaviours
  • Gang involvement
  • Criminal behaviour
  • Low level sexualised

Our homes accommodate no more than 4-5 service users, are of a high quality and are  thoroughly inspected quarterly. The service includes two on-site staff members whose role is to manage risk, work in partnership with our young people’s professional support circle, family members, monitor our young people’s activities and pay particular  attention to supporting our young people with day to day living to ensure they grow and  develop in required areas. Our staff are trained to de-escalate and or seek to prevent all  potential situations and incidents before they occur. 

All of our services are linked to our Emergency out of hours/on call team. Each service  has 2 manned out of hours contact numbers, senior managers are able to visit the service in an emergency if deemed necessary.  

AIMS 

The aim of the service is to: 

  • Deliver a service where young people feel safe, secure and valued.
  • Provide quality support for our young people with autism, learning disabilities, behaviour that is challenging and emotional needs, empowering them to identify strategies that enable them to manage effectively when faced with difficulties and  create a shift to achieve independence and become positive members of the  community. 
  • Deliver and maintain an outstanding service that will improve and sustain the overall quality of life, both physical, emotional and mental wellbeing.
  • Address wider support needs and build connections and opportunities necessary for personal development and achievement in every area.

OBJECTIVES 

In relation to young people: 

  • To ensure that the service is delivered within a person centred and equality framework. We want an environment free from prejudice and discriminatory behaviour and for our young people to feel respected and retain their right to make informed choices. 
  • To provide young people with accommodation that is encouraging of personal choice, appropriate, stable and has a ‘homely feeling’.
  • To create an environment where young people are listened to and are comfortable to express their wishes, needs and feelings.
  • To maintain and support young people in promoting and developing their health, educational and personal development needs.

 To encourage and enable young people to maintain/develop positive  relationships with their peers, family/carer networks and the wider community 

  • To respect young people’s privacy and confidentiality
  • To ensure young people have the skills and knowledge to access support services in the wider community
  • To place young people’s needs at the heart of service design and delivery and to encourage young people’s feedback and involvement in decision-making processes and house activities 
  • To have zero tolerance of any alerts of a safeguarding nature. Progress to be reviewed regularly
  • To record and evidence the outcomes achieved through our services.
  • To ensure and be able to demonstrate a comprehensive quality assurance monitoring policy throughout the organisation.

SEMI-INDEPENDENT LIVING 

These provisions are set up to provide a small group living environment in order to  provide a homely setting and allow for more individual support. The small groups allow  the service users to receive close supervision, build up a range of quality skills for  adulthood, eliminate the issues of lack of engagement, whilst ensuring the young  person does not feel overwhelmed and or crowded. Down time/self-time is encouraged and we help young people to create a personalized relaxation kit that they can use. All  young people benefit from their own spacious bedrooms, fully furnished and specific  services will offer en-suite arrangements. Our supported living services are suitable for  those who require well-structured living options with appropriate boundaries. 

STAFFING 

Our staff understand that to succeed in supporting young people they must develop  strong partnerships with local authorities, professionals, parents/carers/other relevant  adults and aim to achieve a reputation of excellence in all aspects of our service  provision.

We aim to establish positive and effective working relationships with our young people in order to engage them and ensure they are kept safe. 

We will ensure to participate in relevant network meetings/case conferences and  provide comprehensive written/verbal information as requested. 

Our staff will ensure to adhere to the policies and procedures in place at the  organisation and will deliver services within an equal opportunities framework and  actively demonstrate anti-discriminatory working practices. 

Our staff members receive regular training, supervision and yearly appraisals.  Examples of training we provide internally and externally are:

All our staff members are recruited within the safer recruitment framework. All staff have  a current enhanced DBS check and are required to have a minimum of 5 years of  experience. All our senior members of staff have up to a NVQ Level 5. 

Space – We look at how the environment the young person stays or frequents in  impacts on them as a person. 

Staff training – We believe the more informed we are on the needs of our young  people and on specific specialist areas, we can better support them. 

Relationships – We recognize that for our support to mean anything to our young  people, we need to build meaningful connections and memories with them. 

Feedback – Hearing our young people’s wishes and views are important in ensuring  the service and support continues to meet their needs. 

Psychological – We recognize the impact of past trauma and we understand the importance of our young people knowing the connection between thoughts, feelings and  behaviours.

Our key workers will use creativity, think holistic and be inclusive when supporting our  young people. The young person receiving care services is an active participant within  the planning, provision, and evaluation of activities within the residence and the wider  community as a whole.  

The allocated key worker will ensure consistent care and development for each young  person. All young people will have regularly reviewed and up to date support plans, reflecting their own individual needs, wishes and aspirations. There will be regular  discussion on the progress and focus of the ongoing plan. Statutory reviews are  organised and chaired by the young person’s local authority of residence.  

Internally, the progress of each young person will be monitored at core group team  meetings. This will usually take place on a monthly basis and is chaired by the Service  Manager and attended by staff actively involved in the young person’s care.

Base package: 

This is an all-inclusive package (low needs) and includes support in the community as  required. Additional support can be purchased to supplement the package.

    QUALITY ASSURANCE 

    We have in place various systems to assist in setting and achieving targets. We monitor  and review our performance and act on findings, enabling our organisation to  continually improve business quality and performance in the best interest of our young  people. 

    We believe that our young people have the right to demand the highest standards of  service from us and we continually monitor the expectations of the local authority  through our organisation’s assurance plan. 

    We regularly review our young people’s individual care plan. This care plan review  process involves the young person and relevant professionals. The aim of the review is  to ensure that all previously identified needs are being met and any newly identified  needs or changes in circumstances are taken into account when revising the care plan. Our young people are invited to provide consultation to our organisation through our  feedback on services formally on a quarterly basis with their support/key worker, and  informally during house meetings which take place fortnightly. 

    Our organisation is developing our quality assurance systems and is working towards  achieving relevant accreditations.

Key Working

A key working system is incorporated into our ways of working as good practice. All  individual work carried out will be focused upon recognising the importance of being  sensitive and responsive to the needs of our young people. In order to achieve this, we  apply a person-centred approach and incorporate the P.I.E. (Psychologically Informed  Environments) Model. When applying this model, we focus on the five principles:

Recommended Support

We will occasionally recommend additional support that should be purchased in order to  meet a young person’s specific needs (medium–high needs). These recommended  support hours can be negotiated and altered allowing a cost-effective placement/support package to be formed. The recommended support will be added to  the base package.

Admission Criteria

Ondemand Care Supported Living accommodates young people that are making the transition from residential or foster care to independence. We consider all young people from different backgrounds and are firm believers in equal opportunities. Every young person will have a needs and risk assessment completed when referred to the service.  We are able to accommodate young people requiring emergency placements at the service, but this would follow the same admissions procedure. We will not consider referrals for young people where we could not meet their needs effectively. We do consider the compatibility of young people sharing when looking at referrals and young people may be involved in this process.

Base Package

Ondemand Care Supported Living accommodates young people that are making the transition  from residential or foster care to independence. We consider

On agreeing to place a young person on our base package support plan, you agree and  understand that the young person will receive only 5 hours per week 1-1 support over  two weeks duration for the following tasks: 

  • Support with registering at local GP 
  • Support with registering at local Dentist 
  • Support with registering at local Optician 
  • Support with Social/local Integration 
  • Raising awareness of transport links 
  • Raising awareness of local universal service providers 
  • Raising awareness of banks, libraries, and areas of interest 

**Additional Support can be purchased to compliment the base package.
